Measure Description Score/Value
Instrumental Activities of Daily Living (IADL) [1] Measures the level of independence in activities required to maintain independence in the community (i.e. meal preparation, shopping, making telephone calls, money management).

Consists of 7 questions with the scores ranging from 0 to 14.

A higher score indicates less need for assistance

Medical Outcomes Study (MOS) Physical Health Scale [2] Measures a broad range of physical function, assessing the level of limitation with basic activities such as bathing/dressing to vigorous activities like running.

Consists of 10 questions with the scores ranging from 0-100.

A higher score indicates a higher level of physical function.

Karnofsky Self-Reported Performance Status Scale [3] Measures the patient’s self-report of physical function with a one-item scale ranging from “normal” to “severely disabled.”

Consists of 10 questions with the scores ranging from 0-100.

A higher score indicates a higher level of physical function.

Comorbidity: Physical Health Section (Older American Resources & Services Questionnaire (OARS) [4] Comorbidity scale consisting of 13 common comorbid conditions. The number of comorbid conditions are summed. Hearing and eyesight are also rated by the patient.

If the patient has the comorbid illness they will indicate the degree to which it interferes with their daily activities.

Percentage of Unintentional Weight Loss in Last 6 Months Measures the % unintentional weight loss in the past 6 months.

If yes, how much?  
Psychological Status: Mental Health Inventory (MHI)-17 [5] Measures the psychological state of patients via 17 questions regarding how the patient has been feeling in the past two weeks.

The measure yields 3 global scores of psychological distress, psychological well-being, and a total score on a scale of 0-100%.

A higher score indicates better mental health.

Social Functioning: Medical Outcomes Study (MOS) Social Activity Limitations Measure [6] Measures level of social functioning by evaluating the extent to which physical and emotional problems interfere with social activities.

Consists of 3 questions with scores ranging from 0-100.

A higher score indicates a better level of social activity.

Social Support: Medical Outcomes Study (MOS) Social Support Survey [7] 12 items used to measure the patient’s perception of their social support ranging on a scale of 0-100%.

A higher score indicates better social support.
